GENERAL SUMMARY OF DUTIES:
Responsible for providing Care Management (CM) services for patients following established standards and practices. CM is a comprehensive, patient-centered, and team-based approach to managing chronic conditions and improving health outcomes. Displays responsible behaviors, communicates effectively to others and functions as a member of the healthcare team under the direct supervision of the physician.
REQUIREMENTS:
Louisiana LPN license and IV certification are required. This position requires the ability to work between the hours of 7:30 am and 6:00 pm.
