Description:

The CareSight Nurse Practitioner and Physician Assistants lead a care team that consists of Registered Nurses (RN) and Community Health Workers (CHW). They work closely with the patients’ primary care physician.

Position Essential Functions:

• Provides in-home medical care to identify and treat health issues in collaboration with the patients’ primary care physician
• Leads team that includes RNs and CHWs
• Collaborates with RNs and CHWs in the development of an integrated patient centered care plan
• Supports patient and caregiver goals
• Communicates effectively with entire care team including the patients’ community primary care provider
• Travel required
• Required:
• Must be fully COVID vaccinated
• Holds active, unencumbered New York State license
• Active DEA license
• At least two years of clinical experience delivering care to complex patients preferred
• Active, unrestricted New York State driver’s license

Pay: $115,000.00 - $118,000.00 per year
