SENIOR ADVISOR CASE MANAGER (Staten Island) Job at Hamilton Madison House
Under the sponsorship of Hamilton-Madison House and in partnership with The Community Agency for Senior Citizens (CASC) assist Staten Islanders by bringing together community resources to provide case management services. For adults age 60 and older in order to identify client’s needs and service preferences, and work with client (and family, when appropriate) to develop, implement and oversee a care plan to meet mutually agreed upon goals. We do this by working with the older adults, respecting their right to make decisions for themselves, honoring their life experiences and heritage, and recognizing the importance of friends, relatives and community to create supportive environments for people as they age.
Responsiblities:
- Conduct detailed intake and in-home assessments and re-assessment of clients, including closing cases as appropriate
- Develop individualized care plans based on client needs and preferences, and in collaboration with client’s family member(s) and CASC team members; provides ongoing monitoring and updates for care plans
- With program supervisor, authorize EISEP homecare, housekeeping and home delivered meals as appropriate, based on client eligibility guidelines
- Maintain up-to-date knowledge of and working relationships with community resources
- Establish service linkages, make necessary referrals and monitor service delivery to successfully implement individualized care plans
- Provide benefits & entitlements counseling and assistance applying for and follow-up to ensure receipt of benefits & entitlements
- Provide advocacy services for and on behalf of clients
- Maintain up-to-date case records on all clients, including completion of necessary statistical documentation
- Maintain familiarity with all relevant DFTA standards, including updates to standards and procedures
- Point person for daily client intakes via rotating schedule
Minimum Qualifications
- Master’s Degree in Social Work or related field
- Minimum two-year work experience in a social service organization
- Experience working with older adults preferred
- Fluent in Chinese Mandarin preferred
- Excellent written and verbal communication skills, detail-oriented
- Collaborative: works with others in a spirit of trust and teamwork to establish and achieve common goals
- Sensitive to needs of older adults
- Respects differences and values diversity; accommodates linguistic and cultural differences; works well with colleagues and clients with diverse ethnic and cultural backgrounds
- Self-disciplined: manages own time, meets deadlines, works well under pressure, and operates autonomously to achieve key outcomes Analytical: reviews options and considers implications prior to making decisions, develops systematic and logical procedures, presents clear and logical solutions
- Must have a car or use public transportation for travel for work
Hours: Monday to Friday, 35 hours/week
