VNA Homecare Clinical Coordinator Job at Cooley Dickinson VNA & Hospice (CDV)

MAIN FUNCTION\:
The VNA Nurse Coordinator builds, establishes, and maintains a strong referral relationship with Cooley Dickinson Healthcare inpatient and outpatient service lines. The VNA Clinical Nurse Coordinator works closely with case managers and social workers to ensure all patients who would require and benefit from VNA & Hospice services have access to those services.
This position reports to the Director of Patient Care Services and operates within established organizational and departmental policies and procedures.
Qualification:
MINIMUM REQUIREMENTS\:
- Graduate of an accredited school of nursing required
- Bachelor’s degree or equivalent preferred
- Current Licensure in Massachusetts as a professional nurse required
- Current American Heart Association BLS certification required
- Experience in business, sales and/or marketing preferred
- Experience preparing and providing education programs preferred
- Three to five years’ experience working in Hospice and Homecare preferred
- Valid driver’s license, a good driving record, and reliable transportation required
- Demonstrated excellent interpersonal, organizational, and communication skills required
